Between February 26, 1904 and September 30, 1979, the Canal Zone Government, administered by the United States, registered all births that occurred within the Canal Zone.   • Compilation of the required personal information: full name at birth, date and place of birth, and the full names of both parents.
  • Verification of valid identification documents accepted by the Department of State.
  • Submission of the application and payment of the official government fee.
